Tax Information: Important Health Coverage Tax Document (1095-C FORM)
According to new Federal requirements, you may receive a copy of your Form 1095-C upon
request. This means that Asheville City Schools is no longer required to automatically send you
a 1095-C.
If you would like a copy of your Form 1095-C, please access our benefits portal at eBenefits and download
your Form 1095-C electronically.
To download your 1095-C, please follow the steps below:
- Log in to your eBenefits account.
- Once logged in, hover over the “Benefits” tab located in the top right-hand corner of your screen.
- Under “Benefit Reports,” select “1095 Tax Documents.”
- You may then select the appropriate year to download and print your Form 1095-C for your records.
Alternatively, if you need us to mail your Form 1095-C, please contact us as noted below. We
will send your form no later than 30 days after we receive your request.
If you are no longer employed by Asheville City Schools or you retired in this past year, you will receive
your Form 1095-C in the mail.
